Medical & Pharmacy

Cohere Beauty offers medical coverage through United Healthcare (UHC). The health plan offering is HSA eligible meaning if you choose to enroll, you will also have an opportunity to set aside additional pre-tax dollars to cover medical, dental and vision expenses for you & your dependents.

 

Cohere Beauty also makes a contribution to your HSA to assist in lowering your overall healthcare expenses.

  • UHC $3,400 HSA Plan Annual Deductible: $3,400 Individual / $6,800 Family; OOP Max: $6,750 Individual/ $13,500 Family. All services are subject to deductible and coinsurance.

Flexible Spending Accounts

Healthcare FSA: The Healthcare FSA allows you to set aside pre-tax dollars to cover eligible medical, dental, and vision expenses. All elected funds are available to spend on the first day of the plan year. This option is only available to those NOT covered by the UHC $3,400 HSA plan.


Limited FSA: Limited FSA's allow you to set aside pre-tax dollars to cover eligible dental and vision expenses (medical expenses are reimbursed through the HSA). All elected funds are available to spend on the first day of the plan year. This option is only available to those covered by the UHC $3,400 HSA plan.


Dependent Care FSA: For those with dependent care responsibilities, the Dependent Care FSA offers a convenient way to use pre-tax dollars to cover eligible childcare expenses. This benefit is designed to ease the financial burden of dependent care services, providing flexibility and support for working parents. This option is available to all benefit eligible employees, regardless of medical enrollment.

Health Savings Account 

Health Savings Account: If you’re enrolled in the UHC $3,400 HSA PLAN, you’re eligible to contribute on a pre-tax basis to a Health Savings Account (HSA) to cover eligible medical, dental and vision expenses for you and your dependents.​ The funds you contribute to these accounts never expire and will stay with you until you spend them, even if you leave the health plan!

Cohere Beauty will also make a contribution to your HSA based on your level of coverage:

Employee Only                         $400

Employee + Spouse               $1,000

Employee + Children             $1,000

Family                                      $1,000

Cohere Beauty offers NEW dental coverage through Sun Life. Members are offered a choice of two Dental PPO plans: 

  • Dental Low Plan: Annual Deductible: $50 Individual / $150 Family; $1,000 Annual Maximum (per covered individual): Preventive: No Cost: Basic & Major: 50%; No Ortho Coverage

  • Dental High Plan: Annual Deductible: $50 Individual / $150 Family; $2,000 Annual Maximum (per covered individual); Preventive: No Cost, Basic- 20%; Major- 50%; Ortho (up to age 26)- 50% up to $2,000 Lifetime Max

Evidence of Insurability (EOI) 

As a reminder, EOI forms are required under the following circumstances:

  • LTD and STD Coverage:​

    • If you did not elect coverage when initially eligible

    • If you elected coverage when initially eligible, but have since dropped it for 31 days or more

​​

  • Voluntary Life AD&D

    • Employee New entrant:  If you elect coverage when you are initially eligible, EOI is required only for any amount over $150,000.

    • Spouse New entrant:  If you elect coverage for your spouse when you are initially eligible, EOI is required only for any amount over $30,000.

    • Late entrant: Employees/Spouses who previously declined coverage during their initial enrollment or waived coverage at the last annual enrollment will need to provide EOI for any amount.

    • Employee Annual Enrollment: Employees who previously elected coverage for themselves can increase their coverage by up to $50k as long as the increase does not exceed the Guaranteed Issue amount. If you wish to increase your coverage over the GI amount, you must complete the required EOI form.

    • Spouse Annual Enrollment: Employees who previously elected coverage for their spouse must submit EOI if they wish to increase their coverage further.

****Additional coverage will NOT become active until the 1st of the month following date of EOI approval****
